*MUST ATTEND BOTH DAYS*
If interested in trying out for the squad, please complete:
1. An online questionnaire.
2. A Sports Medicine Packet (signed and stamped by a doctor).
Both forms can be picked in the Athletics Dept. (Student Services Building, Rooms 019 & 021) or downloaded from this website - ’Forms’ section of the home page.
